Medical history:

If we have an appointment and you are able to fill out the medical history of you and your baby ahead of time, we will have more time at our consult for hands-on help and we will arrive more knowledgeable about your situation. There is a free app available for iPad users or you could fill them in online if your computer supports Silverlight.

HIPAA:

This is our current copy of HIPAA. We will bring an e-copy with us and include a copy with our visit summary/care plan.
